Volunteer Forms

In order to keep our children safe, the Southern California Conference has instituted policies that require background checks for volunteers and a clean record and insurance minimums for drivers.  All volunteers in any capacity (including hot lunch servers and field trip chaperones) must have passed a LiveScan background check, and all drivers for school events who carry children other than their own must meet the minimum insurance requirements and have signed the SCC agreement.
